Shipping CSC Equipment

This is the process for shipping CSC equipment back from an employee out of state.


  1. Email Patricia Turner, MMDC, with the staff member’s name, address, phone number, number of boxes, value and contents of each box, and account number.
  2. CC staff member as well
  3. Patty will prepare the shipping form and email to the staff member. 
  4. Let Patty know the return address and who it needs to go to.
  5. She will select tracking and you’ll get that information once picked up and delivered.
